Applying for a GAP Service Dog

In order to be eligible for a service dog, you must demonstrate a medical need.

  1. Contact the GAP offices. We are easily reached by phone, email, letter or Facebook. We will discuss your individual circumstances and begin determining what type of dog is needed. We will mail you a letter of intent; complete the letter of intent and return it to us with a doctor’s prescription for a service dog.
  2. Once we have received your letter and notification from a doctor, we will send an application. Return the application and a $25 administration fee and your name will be placed on our active waiting list. We will confirm receipt of your application with a letter acknowledging your completed application and your position on our waiting list.
  3. When a dog becomes available, a member of our staff will contact you to set up an interview and home inspection. The interview and home-inspection process is mandatory and integral ensuring the comfort and safety of both you and your service dog.
